Donor
Sperm
It
is the responsibility of the patient to obtain frozen donor sperm
when donor sperm is needed. The Cooper Center for IVF is currently
recommending Fairfax Cryobank, 3015 Williams Drive, Suite 110, Fairfax,
Virginia 22031. Fairfax Cryobank's telephone number is: 1(800) 338-8407.
If you would like information concerning available sperm donors
and fees for the Fairfax Cryobank you should contact the sperm bank
directly. However, we are willing to use donor sperm from any sperm
bank that you may choose. This should be done well in advance of
your IVF cycle. If you have any further questions please feel free
to contact Bob Long, director of our Sperm Donor program, at the
Marlton office.
If
you need to order donor sperm from the sperm bank, you should do
it prior to starting your IVF cycle. Please follow the instructions
below when ordering donor sperm.
1.
You MUST contact the Andrology Department at Cooper Center for IVF
BEFORE you call the sperm bank and inform us that
you are having frozen sperm shipped to us. The phone numbers are
(856) 751-5250 or (856) 751-5575. The exact shipping address is

Call Fairfax Cryobank to obtain information about the characteristics
of the sperm donors available.
3.
Make your selection from the sperm banks list of donors and arrange
with them to have the sperm shipped to our Andrology Department.
You will arrange for payment of the sperm directly with the
sperm bank.
4.
If you use Fairfax Cryobank you should have three vials (dosages)
of frozen sperm shipped. This should be a sufficient amount for
your IVF cycle.
5.
Contact the Andrology Department at (856) 751-5250 after you have
placed your order to confirm that we have received your shipment.
The
andrology lab will store your frozen donor sperm until it is needed
for your IVF cycle. If you should need any assistance or any further
